Process to Register Birth Certificate in Uttar Pradesh UP Online e-District Citizen Service

UP e-district project which have the official website is providing information to get the birth certificate. Using this facility the portal has made a section from where applicants can download the request form and then can submit through following official process in state.

In legal framework it comes under the Indian Constitution law Births and Deaths Act, 1969 as per registration. Under this the Uttar Pradesh UP govt has built setup for the registration in according to the district, tehsil, village or town wise.

Citizens will have to get the application / registration form as per the below mention process. The registration form needs to send after filling with required information within 21 days of birth to the local authority. Take the all true copies / documents / records of birth from concerned hospital. Or registration can be done as per the revenue authority order and police verification.

How to Download Application Form and Filling Process

NOTE – Before going to submit the registration form must have to pay the fees of application, and affidavit if you are doing registration after the 1 year of birth along with the birth documents from concerned hospital. Here is the process mention below to get the registration form and filling system:

  1. Citizens have to go on the website where need to find the option of “Birth Certificate”
  2. This page will send you in which is a PDF file format of registration form download it.
  3. Now need to fill the application form with the required details as per following:

Select the area type (village / town)

Full name of applicant

Gender of baby (boy / girl)

Father’s name of baby

Mother’s name of baby

Baby’s date of birth

Place of birth with complete address of govt hospital (district / tehsil / block / panchayat)

Your complete postal address

Nationality of baby’s father

Nationality of baby’s mother

Region of baby (hindu / muslim / sikh / Christian)

Complete Permanent address

Mobile number

Date of registration

Place of registration

Registration number / card number

Tick on the yes / no option for application fees

After done with these above process you need to submit the form as per above given process. The department will provide you a certificate which will works as the date of birth proof for citizens.

Through a Citizen Service Center (CSC)

CSCs are the cyber cafes those are authorized to perform the task of the Nagar Nigam operator. They are provided with all the guidelines & forms to fill the registration form in accordance to the information provided by the citizen.

Once an application is submitted an SMS is sent to the citizen’s mobile to intimate the same.

A separate account is maintained through which the CSCs collect the payment from the citizen. This is then later submitted to the Nagar Nigam.

*Note: Once the certificate is generated and is made available on the site, the citizen can collect it either from the site or from the CSC or from nagar nigam service center.

Obtain Birth Certificate (New – Fresh and Duplicate Copy)

  • Get the mentioned above both of services the citizens of UP state will need to follow the process as the given below and through this they have to make registration to get the certificate service:
  • The applicant will have to visit in his / her nearest common service center (CSC) where he / she will have to submit request in prescribed format of application form (which is also available in the CSC office) to obtain above mentioned certificates in Uttar Pradesh.
  • The applicant should have to submit his identification related documents of birth / death certificate from the concerned hospital / gram panchayat officer / panchayat secretary.
  • After providing all details and documents the common service center (CSC) operator will fill the e-form for making the online registration of birth certificate in rural areas of Uttar Pradesh state.
  • The CSC operator will scan all the supporting documents and papers provided by the applicant. The operator will check and verify all the details given by the applicant and in case all information is correct the operator will take digital signature and thumb print.
  • After that the registration through electronic mode will send to the concerned competent officer by the common service center. For the gram panchayat the concerned block assistant / block officer will work as the competent officer and through them the process decantation of all requests will be done.
  • In the gram panchayat level the concerned gram panchayat secretary / gram panchayat officer / block officer will submit the verification report regarding to the valid documents submitted by the applicant with the application form.
  • The complete process will be done through the electronic mode within fixed duration and in case of delay in service according escalation matrix further process will go to get the birth certificate in Uttar Pradesh.
